What is D-Dimer Test
D-Dimer is a fibrin degradation product- a small protein fragment released into the blood when blood clot (thrombus) is broken down by fibrinolysis.
In other words, when the body forms a blood clot (Thrombosis) and then attempts to break it down, tiny protein pieces are produced. These pieces are what we call D-dimer.
Therefore, a high D-dimer level indicates that a blood clot was present, and the body\\\'s natural systems are actively trying to dissolve it.
When is this test performed?
It is primarily used in emergency situations or when a physician suspects conditions such as:
- Deep Vein Thrombosis (DVT): Blood clots in the deep veins, usually in the legs.
- Pulmonary Embolism (PE): A blood clot that travels to and blocks blood flow in the lungs.
- Unexplained pain in legs.
- Sudden swelling (Edema).
- Sudden shortness of breath or chest pain.
Interpretation of the Results:
If the result is normal (Negative): It highly rules out the presence of an active blood clot. ( < 0.5mg/L or (<500ng/ml)).
If the result is high (Positive): This does not definitively confirm a blood clot! It simply means there is active clotting and clot-breaking activity happening in the body. This must be confirmed using imaging techniques (such as a Doppler ultrasound or a CT scan).
Benefits of the Test:
- It streamlines the diagnostic process, helping doctors determine whether a patient requires further, more invasive testing for blood clots.
- It provides reassurance or serves as an early warning sign before a potential complication worsens.
- An elevated level indicates that your body’s anti-clotting system was recently activated to dissolve a clot.
Causes of False Positive? (Elevated without a clot):
D-dimer levels can rise due to several factors unrelated to an actual dangerous blood clot, including:
- Pregnancy (clotting factors naturally increase during this time).
- Advanced Age (the elderly naturally have higher baseline levels).
- Severe Infections and Inflammations.
- Post-surgery and Trauma (the body naturally forms and breaks down clots during tissue healing).
- COVID-19, liver diseases, and various cancers.
- Even high levels of physiological stress can elevate it.
Laboratory Note/ D-Dimer test is performed using citrated plasma (Plasma citrate)—typically collected in a light blue top tube.
Hemolysis (ruptured red blood cells- Red plasma) and Lipemia (excess fat in the blood sample) can negatively impact and distort the accuracy of the result.
The test does not pinpoint where a blood clot is located; it simply tells us that the body is currently involved in a physiological battle of clot formation and destruction.
